Efficacy and Safety of Robotic Dor Fundoplication on Severe Gastroesophageal Reflux Disease in Patients With Scleroderma

Systemic sclerosis (SSc) is a disease that affects the gastrointestinal tract resulting in its atrophy and fibrosis of smooth muscles. Approximately 80% SSc patients develop both gastroesophageal reflux (GERD) dysphagia. The nocturnal GERD can cause regurgitation aspiration, which further aggravate pulmonary from SSc. Also, their dysphagia worsened by performing standard Nissen fundoplication. Therefore, we aimed to investigate whether Dor fundoplication (a 180° anterior wrap) reduce heartburn without worsening with severe GERD. Five drug-refractory underwent procedure median follow-up 2 years (range: 1-5 years). In all 5 patients, preoperative high-resolution manometry showed significant impairment esophageal motility. Patients were interviewed postoperatively assess for diurnal symptoms, treatment response, status dysphagia, adverse effects surgery. average age was 50 females. Four (80%) reported 90% improvement symptoms since surgery, no reflux, heartburn, or regurgitation, reports sleep at night requiring any more pillows wedges. About 50% decrease proton pump inhibitor dosage after surgery compared before No surgical complication specifically, performed refractory substantially decreases primarily nocturnal, affecting thus improving quality life.
